Preparing for your Visit

1. Paperwork

To save time at check-in, please visit our Patient Forms page to complete the required paperwork online prior to your visit. Any required paperwork not completed prior to your visit will need to be completed in our waiting room upon check-in for your child’s appointment.

2. Pre-Visit Testing: Labs and or Imaging

Your child’s appointment may include an imaging test prior to the office visit. If this applies to you, please remember that this testing is scheduled for a time prior to the scheduled appointment in our office, as discussed over the phone with one of our schedulers.

Please note that the appointment reminder messages sent via email, phone, and text message shows the appointment time at our office with our physician or nurse practitioner, and does NOT reflect the time of any pre-visit testing. Pediatric Surgical Associates is a separate entity, and the facility where the testing takes place does not send out separate reminder messages.

Please call our office at 612-813-8000 if you are unsure or need a reminder of when your child’s testing is scheduled.
