Billing & Insurance

Every effort is being made to keep down the cost of medical care. We believe that our fees for services provided are fair and reasonable- and also available upon request. We are more than happy to help you with an estimate of what your surgical costs may be. Please remember; even if you have insurance coverage, you are still responsible for the payment of your account.

For your convenience, our office has adopted a system in which we submit our charges directly to your insurance company. For this reason, we need to have your complete insurance information on file. Please provide us with all policy numbers, addresses, and any claim forms that might be necessary.

Please remember that some insurance companies do not cover all medical costs. Some pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible, co-payment, or other balance not paid by your insurance company. We will make every effort to help you receive the maximum benefits available to you under your policy. Remember, however, that the primary relationship is between you, the insured, and your insurance company. For patients who do not have insurance coverage, we request that you make the necessary arrangements for payment with our billing department. We are committed to keeping your health care costs as low as possible. In order to do this, we need to keep our billing costs at a minimum. Please help us in the following ways:

• Always bring your current health insurance card to the office.
• Notify us immediately of any changes in insurance, address or phone number.
• Please be prepared to pay your co-pay at the time of service, or if you do not have insurance, to pay for your visit in full.
• Please pay your bill in full when you receive your statement or make payment arrangements with our business office.
