ONLINE APPOINTMENT SCHEDULING
FOR POST-OP APPOINTMENTS ONLY!
Pediatric Surgical Associates now offers the ability to book your post-op office visit through our new online booking service!
Please note that online appointments are for returning patients only who need to schedule a post-operative appointment.
If you need to cancel or reschedule your appointment request, please call our scheduling line directly at 612-813-8000, option 3.
Follow the steps below:
1. Click on Book an Appointment button below to begin.
2. Select a location and find your surgeon’s name. Select an open appointment slot.
Note: Not all physicians perform at all clinic locations. If you do not see your surgeons name, you may have to select a different clinic location.
3. After selecting appointment slot, please complete the required information fields.
Note the following important information regarding the required fields:
Phone number: You must enter a valid phone number (we recommend your mobile number) as you will have to enter a verification code sent via automated call or text as the final step before your request is submitted. Text verification is the easier of the two.
Reason for Visit: Please enter “Post op” in this field.
Additional Info for Provider: Here you may enter any special information you have for the doctor or our office staff. The information provided here will be personally seen by your child’s physician prior to the appointment.
Insurance Information: You will be asked to enter your insurance information (carrier and subscriber number) or you may select “pay with cash”. If you do not have your insurance details on-hand at this time, you may select “pay with cash” however bring your insurance card and photo ID to your appointment and inform our front desk staff that you will be paying with insurance.
4. After all required fields are complete, submit request
You must enter a valid phone number in order to verify your appointment via text message or automated call back. If you need to cancel your request, you need to call our office directly and speak to a scheduler.
5. Once appointment request has been received and screened by our scheduling staff, you will receive a confirmation email.
